Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on February 19, 2020 regarding the center's physical plant. The inspection was initiated on March 4, 2020 and completed on March 4, 2020. On March 4, 2020, the inspector was on site from 10:35am to 11:40am. Interviews with staff were completed and documentation was reviewed.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. In addition to the original complaint, there was also a violation found during the inspection regarding staff records. See the violation notice for more details.

Description: Based on record review and interviews, the center did not ensure two of two staff records contained all of the required documentation.
Evidence:
1. The record of staff #1 did not contain documentation of the following: name, address and telephone number of a person to be notified in an emergency and documentation of any health problems which may interfere with fulfilling the job responsibilities.
2. The record of staff #2 did not contain documentation of the following: name, address and telephone number of a person to be notified in an emergency and documentation of any health problems which may interfere with fulfilling the job responsibilities.

Description: Based on interviews, the center did not ensure areas and equipment of the center were maintained in a clean, safe and operable condition.
Evidence:
The Chickadees classroom, which was serving as the Baby Birds classroom at the time of the local agency's visit, had a cabinet that contained old mouse droppings. The cabinet was not being used however the classroom was in use.
Interviews revealed the following:
1. A local agency conducted an inspection on 2/20/2020, regarding physical plant issues that were reported to their agency.
2. The agency representative wrote that there was an art and craft cabinet located in the infant room that contained old and dry mouse droppings.
3. The center's representative confirmed that there was a cabinet located in the Chickadees classroom which contained old mouse droppings. The center's representative also stated the following: the cabinet was cleaned as soon as the mouse droppings were discovered; the center was unaware that the cabinet had mouse droppings until 2/20/2020; the cabinet had not been used in a very long time.

Plan of Correction: The cabinet in the classroom was cleaned immediately when the droppings were found. The center has a contract with [pest control company] to come and inspect the school every other week to address any ongoing issues. The Director and Assistant Director will train the teachers to check their cabinets regularly and report any findings to the Director and Assistant Director.
